Applying to your program:

(OPTIONAL) Guidelines for Summer Abroad Student

(OPTIONAL) General Recommendation Form - This form may be used for the completion of applications for all Duke-administered programs (except OTS programs and Duke Study in China - see below).

(OPTIONAL) Language Recommendation Form - This form may be used for the completion of applications for Duke-administered programs requiring a language recommendation.

Mandatory forms for summer study abroad for credit:

(REQUIRED-ALL STUDENTS) Summer Authorization and Consent Form (Duke-administered program students due at the time of application; Duke-approved and summer petition program students due by the end of classes in spring 2008.) NOTE: This Statement of Authorization and Consent is specific to summer programs.

(REQUIRED - ALL STUDENTS) Duke Summer Programs Commitment to Participate Form. This forms signifies that you have been accepted to and have committed to participate in a Duke-administered summer study abroad program.

(REQUIRED - ALL STUDENTS) Summer Program Evaluation Form (Due upon completion of the program - please return in electronic or paper form to the Office of Study Abroad.)

(REQUIRED IF WITHDRAWING) Duke Summer Program Withdrawal/Refund Form (policy page and form linked)

DUKE STUDENTS ONLY

(REQUIRED for non-Duke programs only) Transfer Credit Approval Form (Duke-approved and summer petition program students)

NON-DUKE STUDENTS

(REQUIRED - ALL STUDENTS) Third Party Billing Form - Please submit to your study abroad office at home institution at the time of acceptance to your program. They will return it to us directly if applicable.
